In accordance with ORS 651.190, every hospital that is required to submit an Equal Employment Opportunity Data Collection Form to the United States Equal Employment Opportunity Commission or survey data to the National Center for Education Statistics must provide a copy to the Bureau no later than 90 days after the report is due to the Commission or Department. ORS 651.190 and 651.195 and OAR 839-002-0405.
EEO Reports must be to the Bureau by September 3, 2024.
Hospitals must file the required reports via email to CRD.Reporting@boli.oregon.gov. Please be aware that the Bureau is required to post these reports to its website.
A hospital may file a request for an extension of the deadline for filing these reports for good cause by submitting a request via email to CRD.Reporting@boli.oregon.gov. Note that the request must be submitted more than 10 business days before the deadline.
