Disciplinary Action Report

Disciplinary Information

All regulated social workers (RBSW, LMSW, CSWA, LCSW) are governed by the Board’s Code of Ethics in Oregon Administrative Rule Chapter 877, Division 30. The full text of the Ethics Code can be accessed at
Individual licensee records will identify if a regulated social worker has been formally disciplined by the Board. In most cases where discipline has been assessed, you will find a link to a PDF document with the full text of the Final Order imposing discipline. If you cannot find it, or have trouble accessing that document, please contact the Board office in writing at oregon.blsw@state.or.us.

Historic Disciplinary Action Document

This document provides a summary of every public disciplinary action taken by the Board of Licensed Social Workers during the last five years. In chronological order, the document lists the name and license number of the person disciplined. Following each name, the document provides a brief identification of the circumstance(s) raised in the complaint, the Statute(s) or Rule(s) that were violated, and the specific disciplinary action(s) taken by the Board.
