Health insurance
The following are resources who can help you with health insurance and Medicare.

PERS Health Insurance Program (PHIP) — PHIP offers health insurance coverage for you, your spouse, and dependents who meet the eligibility requirements. Visit the

Oregon Senior Health Insurance Benefits Assistance (SHIBA) — SHIBA offers certified, local counselors who can help with Medicare-related questions. Learn more on

Account access
For someone to review your account or act on your behalf in the event of severe illness or death, you will need to provide authorization to PERS using the following forms. These forms are only valid while you are alive; read the next section to find information about what happens in the event of your death.
When you die
A family member, beneficiary, or caregiver must
immediately notify PERS of your death. PERS will request a photocopy of your death certificate.
If you participate in PHIP or the Oregon Savings Growth Plan (OSGP), these entities also will need to be notified.
