Getting started
First, open the questionnaire and read through all the questions so you know what information is required. Begin the questionnaire when you have all your answers and are ready to fill the form out completely.
The information you provide will be used to determine your eligibility and whether your household is invited to apply for HARP, so read carefully and fill out the questions below as accurately as possible. If you are not eligible now, you may be eligible in the future or may qualify for other ReOregon programs and will be notified when they open.
Do not submit more than one form per household. Multiple form submissions will slow down processing of your questionnaire. If you need to correct your form or are unsure if you have already submitted one, please contact the ReOregon Call Center at 1-877-510-6800 or 541-250-0938 or email

Setting up an email address
You will need an email address to fill out your Eligibility Questionnaire and to apply to other ReOregon programs.
If you do not have one, you can use an email address that belongs to a family member or friend, but ReOregon recommends you create an email address for your personal use. This will make it easier for us to stay in contact with you, and to send you important updates.
