The Long-term Care Community Nursing (LTCCN) Program helps individuals, their caregivers, foster home providers and case managers to support Medicaid-eligible individuals with chronic health care needs in their own homes or foster homes.

Referrals and authorizations
Referrals and authorizations for LTCCN are made by each person's case manager. Ongoing services can be provided alongside short-term home health services, which are a different type of community nursing authorized by a physician and funded through the person's medical insurance plan.
Who provides services
The LTCCN service can only be provided by self-employed registered nurses (RNs) or RNs employed by In-Home/Home Health agencies. Both types of providers must have a contract with the Oregon Department of Human Services and be enrolled as Medicaid providers.
What services are provided
LTCCN RNs provide:
- A nursing assessment, medication review and service plan for each individual they serve
- Health education for the individual, caregivers, foster home providers and family members
- Teaching the individual or family members on how to perform nursing tasks
- Delegation of nursing care tasks provided by non-family or paid care providers
- Technical assistance and progress summaries to the case manager regarding the person's health care needs
- Care coordination with the person's medical and ancillary health providers
