Community rehabilitation programs who want to join the Oregon Forward program must complete and submit this form.
Documentation of disability
This form, signed by a medical professional, is an "Acceptable Disability Documentation Source."
Competitive employment statement
Participants must complete this form every two years for each employee with a qualifying disability.
Price approval
Oregon Forward Contractors must submit this form with their proposal to the Public Agency customer, signed by a company representative. The information in the form must include prices for all services required by the contract. The completed contractor-signed Price Approval form must accompany the request for the price determination to DAS. The submission to DAS should include completed DAS-approved costing workbooks for all prices proposed, the contract or Statement of Work, and any other documents submitted with the Oregon Forward Contractor's proposal.
Oregon Forward annual status reporting
All participants must use this form to report information to State Procurement Services within 120 days of the end of their fiscal year.
