Step 1: Apply for a license

.
Don't forget to:
- Send a complete application (page 1, 2, and 3)
- Submit an experience verification form for each employer
- Submit any additional documentation required (i.e. high school diploma, transcripts, copies of other licenses, copy of picture)

Plumbing Experience Verification Form
This form is required for each employer listed in the employer experience section (step 4) of your application. You must list your experience in the required work categories in
OAR 918-695
Step 2: Schedule the exam
After your application is received and approved, the division will mail you a letter of authorization that details how to schedule the licensing exam.
After the exam is completed, you will be notified by mail of the results no later than two weeks of taking the exam. If you pass the exam, your license will be mailed to you.
Application Checklist
A complete application must include the following:
- A complete Journeyman Plumbing License Application, with affidavit signed (step 8)
- A complete Plumbing Experience Verification Form for each employer
- Additional documentation:
- Proof of completion of an out-of-state apprenticeship program recognized by the State of Oregon
- Official transcripts of classroom training
- Proof of a high school diploma, GED, or equivalent. A college degree will substitute
- A Passport-style photo (2"x2") with the applicant's name on the back
- Payment of fee
