Record Inquiry Accounts


Entities those who qualify under ORS 802.175-802.191 to receive personal information, to apply for a DMV Record Inquiry Account.  Established account holders may request records via fax or by ordering online.  Records ordered are returned by fax or mail.  Account holders are billed for DMV services on a monthly basis. How to Apply

  • Fill out a Record Inquiry Account Application;
  • Attach required documents (see application);
  • Include the $70 fee; and
  • Mail to:

Automated Reporting Service

DMV's Automated Reporting Service is available to Record Inquiry Account holders with drivers under their employment. This service produces and mails the employer a Certified Court Print driving record when a conviction, accident, suspension, cancellation or revocation is posted to one of their employee's driving record.
