The Providence Choice Medical Home Plan
Providence Choice is a Medical Home plan. A medical home is a clinic staffed by health care professionals who work together as a team to give you coordinated, connected care to keep you as healthy as possible and living well.
 
Led by your primary provider, this team coordinates all of your health care, including referrals to outside providers or specialists when necessary. Each member of the team stays informed about and actively participates in all aspects of your care.

A medical home can be a clinic or group practice. Medical Homes in Providence Choice are clinics and practices that focus on each patient's needs to provide coordinated care efficiently and effectively through the medical-home model. 
 
When you enroll in Providence Choice, you need to choose a medical home to ensure you have access to the full benefits of the plan, including claims paid at the medical home benefit level.

If you have established a relationship with a primary care provider and are considering Providence Choice, contact the provider to ask if the practice is part of the Providence Choice network of medical homes. If you don't have a primary care provider, see the list of medical homes in your area and call the medical home of your choice to ask if they are accepting new patients.
 
When you need treatment outside your medical home, your medical-home provider will refer you to the appropriate specialists and facilities. These specialists and facilities are considered in-network with the referral. All hospitals are considered in-network for necessary emergency care.
 

Medical Homes in Oregon
Medical homes may go by another name in Oregon - a Patient Centered Primary Care Home, or PCPCH.
 
Many clinics and practices in Oregon already follow the medical-home or PCPCH model to meet the needs of their patients. As a single team of health professionals, they focus more on prevention and delivering needed care to save time, money and lives.
 
These groups can now be recognized by the Oregon's PCPCH program for their commitment to better health, better care and lower costs.
 
Primary care homes are the goal for delivering quality patient care within Coordinated Care Organizations -- the building blocks of health reform in the state. CCOs would be required to include recognized primary care homes in their networks of care to the extent possible, keeping patients healthy, out of the hospital and living their lives.
