I need an account or benefit payment verification
Account verifications are for active/inactive members; benefit payment verifications are account balance statements for retired members.
 
To maintain security on your account, PERS cannot release account or benefit payment information via email.  You may either contact PERS Customer Service directly at 503-598-7377 or toll free 888-320-7377, Monday through Friday 8:30 a.m. - 5:00 p.m.), or you can send a written request to PERS for an account or benefit payment verification (P.O. Box 23700, Tigard, OR 97281-3700).
 
You can also fax the written request to: 503-598-0561).  Please include your full name, the last four digits of your Social Security number, your signature, and date.  You will need to mention that you require an “Account Verification” or  “Benefit Verification” Be sure to include your address or fax number for us to send a response.  You will receive a response within 7-10 business days.
 
 
 
 
 
 
 
 
 
 
 
 
 
 
 
 
I need a duplicate annual statement
You may email, mail, or fax your request to PERS. Please include your full name the last four digits of your Social Security Number and verify your address.  Your duplicate statement will only be mailed to the address on file with PERS. 
 
(If your address has changed you will need to submit a change of address with your employer.  If you are retired you will need to complete a  Name/Address Change/Duplicate 1099-R. form.)

I need an IAP PIN
If you are a new member who has qualified for the Individual Account Program (IAP) but have not received a personal identification number (PIN) or if you have forgotten your PIN, use the "Forgot your PIN" option on the IAP website at:
https://iap.csplans.com/portal/PortalLogin.jsp
 
Please click on the "Forgot your PIN" hotspot, then select "SSN" from the pulldown menu, and enter your Social Security number.  Then click on 'Submit', and you will receive a confirmation that your PIN will be mailed to your address on file with PERS within a few days.

I need an duplicate 1099R
If you are a retiree and need to request a duplicate 1099R you may complete the Name/Address Change/Duplicate 1099-R form and submit it to PERS via fax (503-598-0561) or mail it to the address below.
