Form Name Description
Estimate Request Form to request an estimate of retirement benefits.
Additional Estimate Request Fee Agreement Fee agreement form for requesting more than two estimates in a year.
One-Time Variable Election: Active/Inactive member Transfer from Variable Annuity Account to regular account.
Pre-Retirement Designation of Beneficiary 
(Standard Designation)		 Choose or change your beneficiary before retirement. You will also want to complete an IAP Beneficiary Form.
Special Power of Attorney Power of attorney form to appoint others to request, change information, or make decisions.
