Individual Account Program (IAP) Active Member Forms
The IAP (Individual Account Program) is a separate account from your Tier One, Tier Two, or OPSRP account. You must fill out IAP forms even if you have filled out similiar forms for your pension plan.

Form Name Description
Direct Transfer Rollover Acceptance Roll over benefit to qualified plan.
IAP: Revocation of Spousal Beneficiary Designation Revoke current beneficiary.
IAP: Pre-Retirement Designation of Beneficiary Packet Specify beneficiary before retirement.
