Police, firefighter, and 911 telecommunicator forms

As a purchaser of police officer and firefighter (P&F) units, you select the effective date of your P&F unit payments. Complete the Police and Firefighter Unit Payment Application.

As a police officer and firefighter (P&F) member of the Public Employees Retirement System (PERS), you may elect to make additional contributions to purchase up to eight units of additional benefits. To purchase units complete the Police and Firefighter Unit Purchase Election at Retirement.

If you were a police officer or firefighter employed with a non-participation Oregon PERS employer before you became an Oregon PERS members, you may be eligible to purchase retirement credit for that service. Complete the Certification of Nonparticipating Oregon PERS Employer for Police or Firefighter.

If you were a police officer in another state, you may be eligible to purchase up to four years of retirement credit for that service. Complete the Certification of Out-of-state Police Service.

Rollover Eligible Distribution. Required only if you elect a lump-sum payment option and you wish to rollover the benefit.

911 operators can retire if they have 25 years or more of creditable service as a 911 operator. To ensure you meet this requirement, complete the Certification of 911 Operator Service form, and submit it to PERS.
