HSIS CAMEO-Ready Data Files Overview
The CAMEO (Computer-Aided Management of Emergency Operations) Suite assists emergency responders and planners with hazardous material planning.
These are free downloadable programs available from the Environmental Protection Agency (EPA) website at http://www.epa.gov/ceppo/cameo/.
The OSFM creates CAMEO files containing data from the Hazardous Substance Information Survey. These data files can be imported directly into the CAMEO database.
Emergency responders and planners are invited to complete the Hazardous Materials Information Request form to request the HSIS-CAMEO-ready data files for a particular Hazmat Team, Fire District, county, or zip code.

| Hazardous Substance Information System (HSIS Software) |

The Hazardous Substance Information System (HSIS software) is an interactive application that contains data from the Hazardous Substance Information Survey.
The HSIS software includes facility demographic data, facility specific chemical data (including quantity and storage), and chemical data. It also includes on-screen help and a user manual.
Secured Information
Because some of the information reported by facilities has been determined to be confidential and secured, two versions of the HSIS software are produced: The Public version and the Fire Service version.

How to Access the HSIS Software
The HSIS software is available on CD or via an internet download. The software is updated on a monthly basis. Complete the Hazardous Materials Information Request form to request the HSIS software via CD or the internet. CD requesters will be mailed a CD. Internet download requesters will be provided with a username and password for downloading the file.
Once you have a username and password, go to this website and open the Instructions. These provide guidance for downloading, uninstalling, and installing the HSIS software. Please read the Instructions for where to save the HSIS.exe file, and how to proceed. Failure to uninstall prior to extracting the HSIS.exe file will result in an inability to uninstall the previous program and an inability to install the new program. Please follow the instructions to uninstall previous versions.
Download HSIS Software
The time it takes to download the software will depend on your connection and computer speed. If you are using broadband (cable, DSL, etc.) you can expect the download time to be 6 minutes or less. If you are using dial-up, please request the HSIS software on CD.

Fire Version Option: The following two databases may be saved to C:\Program Files\HSIS. These databases can then be accesses via the HSIS software, Main Menu. If you choose to use these databases, please do not download them again in the future or you will overwrite the entries you made into them.

MSDS For HSIS Software
The HSIS software allows users with and without internet access to view MSDSs. If you do not have internet access, complete the Hazardous Materials Information Request form to request the MSDS DVDs, and e-mail the form to sfm.cr2k@state.or.us.

| Hazardous Materials Incidents |
|
Since 1986, OSFM has collected Hazardous Material Incident Reports from emergency responders who respond to incidents. These reports are entered into an Incident database.
Hazardous Materials Incident Database
All or part of the Incident Database can be burned to CD and mailed to you. If you want all the incidents for one county, or within one zip code, or for one city, this may be the best way to view the data.
To request the incident database, complete the Hazardous Material Information Request form and e-mail it to sfm.cr2k@state.or.us.
Hazardous Materials Incident Online Searchable Database
Viewing the Hazmat Incidents online is the quickest way to view them. The Hazardous Materials Incident Searchable Database is updated monthly. To access the incident search, click here.
Hazardous Materials Incident Report
To find out through OSFM if a hazardous materials incident has occurred at or near a location, complete the Hazardous Materials Information Request form and e-mail it to sfm.cr2k@state.or.us. Include the Incident ID if known, and the address of the business that is within the area of concern. If the exact address is not known, please provide an address range (e.g. 1000 to 8000 Main Street, Salem).
If a Hazardous Materials Incident is identified as being at or near the address specified, the database report will be converted into a portable document format. It can then be emailed, faxed, or mailed to you.

| Material Safety Data Sheets (MSDS) |

Material Safety Data Sheets are available for many of the hazardous substances in the Hazardous Substance Information System (HSIS software).
How to Access an MSDS Online
Viewing an MSDS online is the quickest way to view it. The MSDS searchable database is updated monthly. To access the MSDS search, click here.
MSDS DVDs
If you do not have internet access, complete the Hazardous Materials Information Request form to request the MSDS DVDs, and e-mail the form to sfm.cr2k@state.or.us.

If you cannot find the information you need online, you can request a customized report. Please complete the Hazardous Materials Information Request form and submit it to our office. Indicate which fields you would like to have included on the report by marking the available fields on the back of the request form.

The data you request can be provided in the most common formats.

Public Report
A Public Report is a report for one business that shows non-secured information obtained from the Hazardous Substance Information Survey. You may request this report by completing the Hazardous Materials Information Request form and e-mailing it to sfm.cr2k@state.or.us. Please include the Facility ID and site address of the business. If the exact address is not known, please provide an address range (e.g. 1000 to 8000 Main Street, Salem).

Hazardous Substance Searchable Database
Hazardous Substance Searchable Database: You can perform a search to find facility and substance information based on the Hazardous Substance Information Survey last submitted by the facility. To find the hazardous substances that a specific facility has on site, you must have the ID number for that facility. This database contains various searches that will help you find the correct facility and its ID number. You can then run the ID Number Search to locate the facility with its associated hazardous substances. This online database is updated monthly.
To request a Public Report for one or several facilities found using the searchable database, or to request this information in a different format, complete the Hazardous Materials Information Request form and e-mail it to sfm.cr2k@state.or.us. Include the Facility ID and site address for each site you are interested in.
Hazardous Materials Incident Searchable Database Hazardous Materials Incident Searchable Database: You can perform a search to find information about an incident that occurred in Oregon. The information in this database dates back to January 1, 1985.
To request a copy of an incident found online, please complete the Hazardous Materials Information Request form and e-mail it to sfm.cr2k@state.or.us.
Material Safety Data Sheet (MSDS) Searchable Database Material Safety Data Sheet (MSDS) Searchable Database: You can perform a search to find an MSDS for a hazardous substance found at an Oregon facility. This database is updated monthly.
The MSDS's in this database is for general information purposes only. The MSDS in this database should not replace the most up-to-date manufacturer's copy.
This search allows you to find an MSDS for specific chemicals reported in Oregon.
NOTE: If you search for a substance such as hydrogen, the resulting MSDS list will include each MSDS in the database that contains the word hydrogen, such as Hydrogen Bromide, Hydrogen Chloride and Hydrogen Peroxide.
