Facsimile of Stamp
809-50-000 Every registered geologist and/or certified engineering geologist shall use a stamp bearing the name of registrant {Name should appear in place name here location in facsimile below}, the certificate number {number should appear where 000 is printed in facsimile below} and the words Registered Geologist or Certified Engineering Geologist.
Please note that after placing your stamp on your document, you may sign through the center space or place your expiration date in the space.
