• Malpractice Report: This database compiled from Negligence Claim Reports filed by insurers of professional liability policies under rules promulgated by the Insurance Division of the Oregon Department Of Consumer and Business Services. • Label Formatted Disk: (CSV format) includes: License Number, Licensee Name, Current Mailing Address. A Data Request Form must be completed.

STATUS

• Active: The licensee practices within the state.
• Inactive: The licensee practices and lives outside the state.
• Emeritus: The licensee is retired from active practice, but may do volunteer work without pay in Oregon.
• Locum Tenens: The licensee practices intermittently within the state, but resides outside the state.
• Limited License: Licensees who have limited licenses are restricted according to the particular type of limited license they have been issued. For a full explanation of all the various types of limited licenses see Limited Licenses Definitions.

• DO: Osteopathic Physician and/or Surgeon
• DPM: Podiatrist
• LAc: Acupuncturist
• PA: Physician Assistant
