Includes Board Order Report (see below) if the licensee has/had legal orders. Cost: 1-4 license verifications: $10.00 per name; 5 or more license verifications: $7.50 per name. Board Action Reports

- Licensing Action Report: A quarterly list of all Board licensees who are currently under any formal Board action. The Licensing Action Report is posted on the Board's web site. Cost: No charge.

By calling this number (971-673-2700 8:00a.m. to 12:00p.m. and 1:00p.m. to 4:30p.m. Pacific Time) a person can obtain the following information about any health care provider licensed by the Board: license number, date first licensed in Oregon, expiration date, license status, whether or not a licensee is in good standing with the Board, mailing address, business address, business phone number, date of birth, specialty, and location of patient records. Limited to 3 names per phone call. Cost: No charge.
Malpractice Search
- Malpractice Search: Information reported by insurance companies on closed claims of malpractice made against Board licensees in Oregon. A closed claim is a claim that has been reported as settled, dismissed, or paid. Cost: $10.00 per name.
- Closed Malpractice Cases Report: Monthly report of newly closed malpractice claims made against Board licensees in Oregon, includes licensee name, license number, city and zip code, name of insurer, description of the closed malpractice case, and closure data. Cost: $15.00 per monthly report.
