Preferred Language Cards

Use These Cards to Tell Providers About the Help You Need.

These wallet-size cards are in Oregon's most frequently used languages. You can print a card in your preferred language. Keep it in your wallet. Show it to your health care providers. Then they will know to:

  • Ask you about your preferred spoken language.
  • Add your preferred language to your medical record.
  • Make sure you have an interpreter for health care appointments.

How to Print the Cards

Set your printer to duplex (double-sided) printing. Use a cardstock for best results. Cut the printed card to size, then fold in half. You can also ask your provider or plan to print one for you. They can also replace it if you lose it.
