- From Eastbound Willamina Salem Highway (OR-22), just past the OR-99 junction veer LEFT on the Dallas/Kings Valley Highway (OR-223) exit.
- Continue on Kings Valley Highway/Ellendale Ave (OR-233) for approximately 3 miles.
- The DMV office is on the Right-hand side of Ellendale Ave just before Polk Station Rd.
Going West
- From Westbound Willamina Salem Highway (OR-22), continue past Perrydale Rd.
- Turn RIGHT on Kings Valley Highway (OR-223) toward Dallas.
- Continue on Kings Valley Highway (OR-233) for approximately 2.5 miles.
- Turn LEFT on Polk Station Rd.
- Turn LEFT on Ellendale Ave (Polk Station Rd dead-ends at Ellendale Ave).
- The DMV office is on the Left-hand side of Ellendale Ave near Polk Station Rd.
