ORS 679.120(4) and ORS 680.075(4) require that all Licensees of the Board advise the Board within 30 days of any change of address. You can do this in a variety of ways: mail, email, fax or submitting the electronic form below. Use of the electronic form is highly encouraged.
All changes must be in writing, we CANNOT accept changes over the phone.
The above forms are NOT to be used for a name change. To submit a name change please send an email to Lisa.Warwick@state.or.us, listing your old name, new name, reason for change and license number.
Please Note: APO/FPO Address changes cannot be submitted using the electronic address change form. Those who need to change their address to an APO or FPO may send an email directly to Lisa.Warwick@state.or.us or the print version of the form may be used.
