Fill out and print a deferral application form. You may also request an application from your local county assessor’s office or the Department of Revenue. The application is two pages, and both pages must be completed. Page 1—Applicant Section - Mark Disabled or Senior at the top of the application.
- Check type of applicant box.
- Fill in your name, Social Security number, birthdate, and age.
- If there is a joint applicant, complete the information on the second line.
- Fill in your mailing address. If your property address is different, fill in the property address.
- Fill in the city, state, and ZIP code.
- Please include a phone number where we can contact you. If you have a message phone number or someone that handles your affairs, please include their phone number.
The county section of the application will be completed by the county assessor’s office. Page 2 – Income Worksheet and Applicant/Joint Signatures - Fill in your name and Social Security number at the top.
- Fill in your income from all sources for the previous year. Include both taxable and nontaxable income.
- Sign and date form at the bottom of the page
Household checklist After completing the application, mail it to your local county assessor’s office. The assessor’s office will forward your application to the Department of Revenue. The department will review your application for completeness and compliance. If we need additional information, you will be contacted by mail or phone. You will be notified in writing if your application is approved or denied. It takes 6–8 weeks for the department to process your application.
