Health Maintenance Tracking

Certain routine examinations and lab work will need to be tracked to make sure they are done on a timely basis. The following are commonly tracked items:

  • Dental examinations
  • Eye examinations
  • PAP smears
  • Mammograms
  • Physician specialist appointments (neurology, ENT, orthopedist, etc.)
  • PCP appointments
  • Physical Therapist evaluations
  • Occupational Therapist evaluations
  • Dietary evaluations
  • Laboratory results
There may be other items tracked, depending on the person's needs. In a community living situation where nursing services are not used, this tracking is usually done by the house manager or medical manager. Where an RN is involved, the nurse will usually want to do his/her own tracking. The following is a sample tracking form.
