What is WISEWOMAN?
The Oregon WISEWOMAN Program provides heart disease, stroke and diabetes screening for medically underserved and low income women at no cost. Contracted and trained Breast and Cervical Cancer Program (BCCP) providers screen BCCP patients for the WISEWOMAN Program. We aim to reduce morbidity and mortality related to cardiovascular disease and diabetes and help women lead a healthier lifestyle.
Why Should I Become Involved?
- Ability to offer heart disease, stroke and diabetes screenings at no charge to your Breast patients as an integrated visit with BCCP screenings.
- Increase reimbursement for office visits, administrative work, lab work, and counseling associated with WISEWOMAN visits.
- Ease of adding WISEWOMAN to BCCP services with 1 enrollment form for both programs and having an integrated data/billing system.
- Receive support to refer your WISEWOMAN patients to the Chronic Disease Self Management Program "Living Well with Chronic Conditions/Tomando Control de su Salud" program
- Gain access to free publications for your WISEWOMAN patients to lead a healthier lifestyle.
- Receive support for lifestyle counseling.
- Learn from other providers on effective heart disease, stroke and diabetes screening and resources for your patients.
- Contribute to a CDC-funded program that improves access to health services for low-income women.
