X-ray Machine Registration
Oregon Administrative Rules (OAR 333-101) require registration of x-ray machines as follows:
Division 101
Registration of Radiation Machines, General License Radioactive Materials, Licensing of Radiation Services, And Accreditation of Hospital Radiology Inspectors
Purpose and Scope
- 333-101-0001 (1) This Division provides for the registration of radiation machines, general license radioactive materials, and for the licensing of persons providing radiation machine, radioactive material, or tanning installation, consultation, servicing, and/or services, and hospital radiology inspectors performing hospital X-ray machine inspections, unless such activities are subject to other Divisions of these Rules.
- (2) In addition to the requirements of this Division, all licensees, registrants, and accredited individuals are subject to the applicable provisions of other portions of these rules.
- (3) As used in this Division, "facility" means the location at which one or more devices or sources of radiation (X-ray, radioactive materials, or non-ionizing radiation) are installed and/or located within one building, vehicle, or under one roof, and are under the same administrative control.

Application for Registration of X-ray Machines ONLY
- Step 1: After a new or used x-ray machine is obtained, an application form to register the machine should be received from the seller. This is to be completed by the purchaser (the new registrant). The completed form should then be sent promptly to the Radiation Protection Services (address is on the application). If an application form has not been received, you can print out a blank application form.PDF file (75K) Facsimile transmissions to 971-673-0553 are acceptable.
- Step 2: The machine information is entered into a database, and an X-ray Machine Registration form is generated then mailed to the applicant. A return mail cover page is enclosed.
- Step 3: Applicant returns this X-ray Machine Registration form along with payment. Checks are to be payable to the Oregon Health Division.
- Step 4: A Validation Certificate is issued and sent to the applicant within approximately 4 - 6 weeks.
- Step 5: Applicant must then POST the Validation Certificate in a conspicuous place, preferably, on or near the x-ray machine that is registered.
