Living Well with Chronic Conditions - A Six-Week Workshop
Living Well with Chronic Conditions (the Chronic Disease Self-Management Program, or CDSMP) is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.

Chronic Disease Self-Management programs also include Tomando Control de su Salud, a Spanish-language, culturally appropriate version, and the Positive Self-Management Program (PSMP), a workshop for people with HIV.

The Oregon Department of Human Services (DHS) supports Living Well/CDSMP workshops in Oregon by providing opportunities for leader training and networking. This is coordinated through Health Promotion and Chronic Disease Prevention programs, Seniors and People with Disabilities, and the HIV Care and Treatment program.
