Enrollment in the Vaccines for Children Program
- Call the Immunization Program at (971) 673-0300 and request a "VFC Recruitment Packet"
- Fill in the Private Provider Enrollment/Profile
- Read and Sign the Private Provider Agreement
- Mail all copies of the Private Provider Enrollment/Profile and the Private Provider Agreement forms using the envelope included in your packet.
If you wish, an Immunization Health Educator can visit your practice to review the enrollment forms with you. Confirmation copies will be sent to you upon enrollment.
What's Next?
You will receive a "VFC Welcome Packet" including your VFC Provider Identification Number (PIN) and vaccine order forms. Fill in your vaccine order and mail back to the Oregon Immunization Program. Your first VFC vaccine shipment will arrive within 30 days. All subsequent VFC vaccine shipments will be delivered within 10 days of receipt of your order
Please note that your clinic will need to comlete and return a "Varicella Certification Form" before ordering Varivax. These forms are included in the VFC Recruitment Packet.
