Patient Safety Improvement Corps 2007/2008

 

The Patient Safety Improvement Corps seeks to improve patient safety by providing knowledge and skills to teams of State field staff and hospital partners selected by States.

 

The training is provided through the Agency for Healthcare Research and Quality (AHRQ) and Veterans Administration (VA) partnership program.

 

Oregon Team PSIC project: Developing RCA-based Event Investigation for use in

Oregon Nursing Facilities 

 

Vision: Reduce unanticipated harm to patients in Oregon nursing homes

 

Goals:

  • Improve patient safety in nursing homes
  • Promote understanding and use of a common language around patient safety
  • Make quality improvement tolls that strengthen event investigation and prevention available to nursing facilities in Oregon

 

Objectives:

  • Development of a replicable protocol/tool for event investigation in nursing facilities based upon Root Cause Analysis process and current patient safety concepts
  • Train nursing facility staff involved in event investigatin in use of protocol/tool
