Restaurant Forms
If you plan to operate a new or previously licensed restaurant, a Restaurant License Application must be submitted to the Local Public Health Authority in your area. Facilities that are being constructed or remodeled must complete the plan review process locally prior to construction. If you plan to operate a food booth at a public event, a Temporary Restaurant License Application and fee (if applicable) must be submitted to the Local Public Health Authority in your area.

A Benevolent Meal Site is defined as a periodic food service operation run by a benevolent organization that provides food to the needy or indigent without charge and does not operate from a permanent kitchen facility.
