Reportable Actions
A certified EMT must notify Health Services, using Health Services' EMS Personnel Action Report Form (E-4 Form) (pdf) within:
Fourteen days of occurrence, if any of the following occurs:
- Disciplinary restriction placed upon scope of practice by the medical director;
- Being named as a defendant in a lawsuit which alleges medical malpractice or misconduct;
- Issued a citation, arrested or convicted for driving under the influence or being admitted to a diversion program;
- Arrested or convicted for any drug related offense;
- Arrested or convicted for any sexual, violent, personal or property crime;
- Restriction or loss of driving privileges;
- Admission to a drug/alcohol treatment program;
- A change in medical director. This notification is not necessary when the EMT is affiliated with an EMS agency that is on file with Health Services.

Thirty days of occurrence, if any of the following occurs:
- A change in legal name;
- A change in home address;
- A change in EMS affiliation;
- Develops any physical disability lasting more than 30 days which significantly interferes with the ability of the EMT to perform all of the duties of the level of certification held by the EMT;
- Any change in mental health which affects the ability to perform as an EMT.

Failure to notify Health Services of any items specified in this rule, may lead to suspension or revocation of an EMT certificate or the EMT being placed on probation.
Health Services may, in its discretion, initiate an investigate based upon being notified of any of the items specified in this rule.
