Strategic Plans
  • HB3486 Report - Strategic Plan to Slow the Rate of Diabetes in Oregon (pdf 6MB) - The 2007 Oregon Legislature approved and Governor Kulongoski signed House Bill 3486, which declared an emergency related to diabetes and obesity. The bill directed the Oregon Department of Human Services (DHS) to develop by 2009 a strategic plan to start to slow the rate of diabetes caused by obesity and other environmental factors by 2010. The plan was to identify actions, including funding and statutory recommendations, that would reduce morbidity and mortality from diabetes by 2015. The plan was developed through a strategic planning process organized and staffed by the Health Promotion and Chronic Disease Prevention Section and Office of Family Health of the Oregon Public Health Division in DHS. The plan will be presented to the 2009 Oregon Legislature.
 
  • Cover of Oregon's Action Plan for Diabetes (pdf)Oregon's Action Plan for Diabetes (pdf) - describes goals, objectives, and strategies for reducing the burden of diabetes in Oregon. The Plan targets four topic areas: diabetes policy and advocacy, diabetes awareness and education, diabetes quality health systems, and access to diabetes data. The plan is intended to guide the activities of the Oregon Diabetes Coalition and local community organizations.

Reports and Guidelines
 
  • Population-based Guidelines for Diabetes Mellitus - "Guidelines for Diabetes Mellitus: Measuring Quality of Care in Health Systems" - developed in conjunction with a statewide advisory committee, these Guidelines have been adopted for use in all of Oregon's managed care organizations to guide quality improvement efforts for diabetes care. A compilation of data specifications to measure each guideline is now available, and a patient version of the Guidelines has been produced.
  • Oregon Diabetes Coalition Fact Sheets
    The Diabetes Data Fact Sheets highlight important information related to diabetes in Oregon. They cover five key topic areas:
    • Disparities and Diabetes in Oregon
    • Diabetes Among the Uninsured in Oregon
    • Diabetes Complications and their Economic Impact in Oregon
    • Youth and the Growing Risk of Diabetes in Oregon,
    • and Gestational Diabetes in Oregon.

  • Keeping Oregonians Healthy (pdf) Keeping Oregonians Healthy cover
    Keeping Oregonians Healthy is a statewide report that summarizes information about the burden of chronic diseases and related risk factors among Oregonians. It provides information about how many people in our state are affected by various diseases or conditions that can cause them. It also reviews differences in rates of these conditions by county, by socioeconomic status, and between racial and ethnic communities in the state. Finally, it describes effective strategies through which we can manage and even prevent chronic disease among the state's population.
