The Oregon Diabetes Coalition is pleased to provide this 2006 Progress Report as a means of tracking our progress in achieving the goals outlined in our Action Plan for Diabetes. Monitoring our progress and success is a collective effort among Coalition partners. Together we strive to make this assessment complete and meaningful by providing information on a variety of diabetes-related outcomes among different populations in Oregon. From year to year this progress report will highlight valuable information from several sources including statewide data systems, health care systems, and special data projects.
