Tools for providers
- Tools that providers can use to support on-going care to their patients. These pdf documents include an Asthma Action Plan that summarizes the doctor’s instructions for self-management of asthma, a questionnaire to be completed by patients with asthma when they come for an office visit to assess control, and a Quick Reference Pocket Guide that contains a brief guide to the stepwise approach for managing asthma, as well as a dosage chart for inhaled corticosteroids. Also included is the Guide to Improving Asthma Care in Oregon, which provides baseline indicators for use by health systems and clinics to monitor quality care:
Patient education handouts
- Brief patient education handouts. Providers can use these brief handouts (all in pdf format) to help explain specific topics to their patients and reinforce key messages of prevention and control of asthma. Some have places for the doctor to write down names of medicine and instructions for using them. Current handout topics are listed below:
