Agency Wellness Grant Program
Description and Application
 
Grant Program Overview
 
The PEBB Wellness Grant Program provides small grant funds to agencies to assist them in promoting employee worksite wellness activities, including but not limited to worksite wellness physical activity and nutrition programs and strategies for implementing best practices, guidelines and policies that support agency wellness. A key goal of the program is to further the longer-term sustainability of worksite wellness programs and contribute to efforts to improve employees’ health status and control health care costs. 
 
The Grant Program Will Help Fund
  • Educational resources: CDs, DVDs, brochures, health-related books and magazines (where not readily available through the State Library Wellness Collection), flyers, posters, health-related pamphlets and newsletters, etc.
  • Wellness instruction (i.e. physical activity, nutrition, stress management, etc.)
  • Exercise aids (hand and ankle weights, yoga/stretching mats; jump ropes, pedometers)
  • Program supports: tee shirts, water bottles, wellness calendars, activity logs/journals, etc.
  • Other resources upon approval
Awards
  • PEBB would like to fund as many agencies as possible through the grant program (see “Application Process” section of this document for information about PEBB technical assistance for applying).
  • Awards will be made to help fund certain costs for agency wellness programs for two years.
  • A committee will review all submitted proposals and notify agencies of grant awards.
  • PEBB will send an award letter outlining approved grant funds to each agency.
  • Awards will be granted on an ongoing basis. However, those agencies that are able to apply early will receive priority.
 
Meetings
  • Agencies that participate in the PEBB Wellness Grant Program agree to send a representative to bi-monthly Agency Wellness Coordinator meetings to share progress updates and experiences with wellness coordinators from other agencies.
  • As part of the grant program, agencies that receive funds agree to participate in an annual meeting hosted by PEBB where all participating agencies present information about wellness programs at their worksites and evaluation results
PEBB will consider the following criteria when allocating funds to agencies that apply for the program
  • Number of employees at agency
  • Presence of staff to coordinate agency wellness program
  • Management support of agency wellness program
  • 2-year agency wellness program plan (includes goals, strategies and action plan)
  • Number of scheduled event/activities and scope of events/activities
  • Clearly identified strategies for health and wellness promotion (including but not limited to worksite wellness physical activity and nutrition programs and strategies for implementing best practices, guidelines and policies that support agency wellness)
  • Estimated agency wellness program costs
  • Commitment to evaluation
  • Willingness to participate in grant program meetings
  • Past efforts to promote worksite wellness and engage employees in personal wellness activities
  • Efforts to create a sustainable agency wellness program
  • Anticipated challenges and strategies for dealing with them
  • Involvement in the Statewide Safety and Wellness Leadership Initiative
  • Involvement of union representatives in agency wellness activities

Application Process
 
The two documents described below must be submitted to PEBB to complete the application process. PEBB staff will provide technical assistance to agencies in preparing these materials as needed, with the goal of supporting all interested agencies in completing their application and receiving grant funds.
 
  1. Fill out the grant application form, including the signed statement of management commitment to agency wellness program and the grant program (signed by the agency head or other high-level agency administrator)
 
  1. Include with the grant application form a 2-year worksite program proposal that includes the following information. This proposal should be no longer than three single-spaced typed pages.
  1. Clearly outlines agency wellness program plan (includes goals, strategies and action plan); efforts to create a sustainable agency wellness program (including strategies for implementing best practices, guidelines and policies that support agency wellness)
  2. Statement of past efforts to promote worksite wellness and to engage employees in personal wellness activities
  3. Identifies staff that will coordinate agency wellness program
  4. Details strategies for involving union representatives in agency wellness activities
  5. Details number of employees at the agency and number of employees that will be impacted by the agency wellness program (estimate)
  6. Details how funds would be allocated to individual worksites
  7. Identifies wellness program goals and strategies and cost estimates
  8. Outlines evaluation methods for the agency wellness program and timeline for submitting evaluation results to PEBB
Deadlines
 
  • PEBB will begin accepting applications starting July 17, 2007.  To be considered for grant funds during the initial award period, applications must be received by September 7, 2007. Agencies that submit their application by this date will be notified by early October.
 
  • Applications submitted after September 7, 2007 will be considered in the second award period and awardees will be notified by early December.
 
  • Other applications will be considered on an ongoing basis. However, priority will be given to those agencies that apply before December 31, 2007.
